Narrative

An employee was descending a fixed ladder from the mezzanine (approximately 6.5 feet) to the ground level when she lost her footing and her grip. She attempted to jump from the ladder to avoid falling and landed hard on her feet, fracturing both heels. She then hit her head on the floor, injuring her head. She was hospitalized.
