Narrative

An employee was using a pneumatic nail gun to build a deck. The nail gun was attached to his tool belt. As he bent down, the safety on the nail gun pushed against his leg. The hammer then swung around and hit the trigger on the nail gun, causing it to fire a nail into his thigh.
