Narrative

An employee was climbing a fiberglass extension ladder tied above to a stanchion post. While moving off of the ladder onto the roof, the employee's foot slipped on the roof deck and his hand slipped off of the ladder resulting in a fall. The employee suffered fractured ribs, vertebrae in the neck and a concussion.
