Narrative

Two employees were changing the grinding hammers on a tub grinder that is used to grind mulch. As one employee was using a sledgehammer to pound one of the grinding hammers, a piece of the sledgehammer chipped off and struck the second employee in the arm, cutting an artery and requiring surgery.
