Narrative

An employee had been prepping concrete forms for removal and was on top of one of the forms. While moving from one anchor point to another, the employee fell off the form. His self-retracting lanyard failed and he fell to the concrete, 16 to 18 feet below, sustaining injuries to his head, hip and wrist.
