Narrative

On 8/16/2017, an employee was changing a band mill saw blade. The wheel of the blade was still turning as the employee removed the plywood covering the pit. The plywood made contact with the mill wheel, and the plywood then struck the employee on the left side of the face. The employee was hospitalized.
