Narrative

An employee and a coworker were moving a loader and a coupler, which was on a pallet. They began prepping the coupler to be attached to a hoist's boom. They partially attached the coupler to the boom when the sling broke, causing the coupler to fall down and strike the injured employee's left shin and foot. He suffered metatarsal fractures and tarsometatarsal dislocations.
