Narrative

An employee was using two hoists to lift a 5-foot section of channel iron that was supporting a 200-pound motor. Hoist hooks supported the channel iron through eyelets. While positioning the hoist, the employee placed his finger through an eyelet. Weight shifted, creating a pinch point between the eyelet and the hoist hook that amputated his finger.
