Narrative

On September 7, 2017, an employee was hoisting a large argon gas cylinder with a chain. As the cylinder was being lifted, the chain started to move and the cylinder shifted. The employee grabbed the valve ring on top of the cylinder and was trying to stabilize it when the first digit of the employee's left thumb was caught and amputated between the chain and one of the ring supports.
