Narrative

An employee was at the top of a fixed ladder mounted on the interior of a 10-foot deep trench box. It was raining and the employee's feet and the ladder were wet. The employee slipped off the ladder and fell onto his back on the plywood flooring of the excavation, sustaining a back injury.
