Narrative

Employees were preparing to adjust the blade on the grinder machine. Product was not completely clear from the grinder which prevented the carriage blade from rotating. An employee climbed up onto the carriage and used his weight to force the carriage blade to move. When it did, the employee stepped off the carriage and his left big and second toes were amputated by the blade.
