Narrative

An employee was attempting to clean the index sensor on a bracket welder because of a machine fault. This did not clear the fault, so the employee attempted to clean the fixture. When the employee cleaned the fixture, the fault cleared and the machine actuated. This caused the magnet to come down on the employee's left index finger, amputating the finger at the second digit.
