Narrative

An employee was getting ready to load a truck. He was behind the winch truck, pulling on the chain that goes to the winch, when the driver in the truck pulled the lever that moves the chain that goes to the winch. The chain and the tail roll of the trailer trapped the employee's finger causing a partial amputation of the right ring fingertip.
