Narrative

Employees were performing pressure services, hydrostatic testing on a blowout preventer. When one of the employees attempted to loosen bolts on the wellhead spool, the hydraulic torque hammer wrench and wellhead spool struck the employee s left hand, partially amputating the index finger.
