Narrative

On 11/13/2017, at 7:00 AM, an employee was using a chain winch to remove a screw auger from a tube in the boiler room. The movement of the auger caused a pulley to turn, and the employee's finger was pinched between the pulley and a protruding bolt head. The employee's right index fingertip was amputated.
