Narrative

An employee was servicing a horizontal panel saw that showed an error code for the score blade position. As the employee manually lowered the score blade, the saw carriage tried to return to the 'home' position. The employee was then caught between the saw carriage and the floor of the facility, suffering broken ribs, a fractured jaw, and lacerations.
