An employee was operating an auto fastening machine when the machine failed to complete a tool change. The employee was attempting to troubleshoot the problem when the sensor cleared and the machine cycled, catching, fracturing, and puncturing the employee's right hand. The machine was not in stop mode at the time of the incident.
