Narrative

An employee was instructing a trainee on how to set up a single-cycle stroke on a press machine. His left hand was adjusting a miss-feed of the lamination when the trainee initiated the cycle. The top section of the die then cycled down, crushing the first three digits (middle fingers) of his left hand.
