Narrative

On December 14, 2017, at approximately 5:00 a.m., a temporary employee was standing next to a door that was slightly ajar. Her index finger was in the door jam when another door was opened, which created a vacuum in the room. The first door then slammed shut on her finger. Approximately 1 inch of her right index finger was amputated.
