Narrative

Equipment was being used in an excavation to stop off a high-pressure gas main. The equipment failed, causing an uncontrolled release of gas. This high-pressure stream shot dirt, gravel, and debris from the excavation area, which struck an employee and caused a shoulder separation, requiring hospitalization.
