On January 4, 2017, at approximately 4:50 a.m., an employee used a stand-up forklift to place a pallet onto a storage rack. He was backing away from the pallet when his right heel was caught and lacerated between an upright behind him and the operator compartment of the lift. He required stitches and overnight hospitalization.
