Narrative

An employee was standing on a 12' step ladder about 8' off of the ground while cutting a pipe. As he was cutting the pipe, the pipe came loose from the fasteners that were holding it into the ceiling. It swung down, knocking his ladder over and causing him to land wrong on his right ankle. He was hospitalized with a right ankle injury.
