Narrative

On January 18, 2018, at 5:40 p.m., an employee was in a boatswain chair greasing blocks when the safety hook came undone, causing the employee to fall 15 to 20 feet to the rig floor. He fractured his right arm and some ribs, requiring hospitalization.
