Narrative

The injured employee was standing next to his forklift replenishing the roller racks (by hand). Another forklift operator passed by and struck the injured employee's forklift, which pinned his left foot between his forklift stabilizing arm and the rack resulting in amputation of two-thirds of his left big toe.
