An employee was holding a 10-inch diameter, 90-degree acid line pipe, while his coworker was using a 4-inch cutting wheel to cut the 90-degree portion of the pipe. After the final cut, a 6-inch pipe came out from the inside of the 10-inch pipe and fell on the employee's knee, fracturing it and requiring hospitalization.
