An employee was performing a quality check on incoming materials on a machine's feed line. A nearby forklift operator was attempting to lift a two-tiered stack of cardboard for loading into the machine when the top tier fell on the employee. The employee's right leg broke in several places, requiring hospitalization and surgery.
