Narrative

An employee was repairing a well. He was connecting a polish rod to a rod string and stuffing box. The box was 3 inches above a pipe wrench that was on the polish rod. The employee's hand was on the pipe wrench when the stuffing box slipped down onto his left index finger, crushing it and resulting in an amputation.
