Narrative

An employee was in the process of installing a metal curb on a roof for placement of HVAC. He was kneeling and reached his hand over a floor hole cover. The floor hole cover gave way and the employee fell 18 feet. The employee suffered skull fractures as well as a broken pelvis and femur.
