Narrative

A employee was troubleshooting a mechanical problem on a stacker. His hand was in the chain/sprocket area when another employee on a different part of the equipment started the stacker. The distal portion of his index finger was then amputated. The stacker was unguarded and not locked/tagged out at the time of the incident.
