A maintenance employee was working from a 6-foot step ladder to repair an overhead door when the pipe wrench he was using slipped off a tension spring and hit his head. He then fell off the ladder, landing on his right side and hitting his head on the dock floor. He sustained two skull fractures and a hairline fracture to the jaw.
