Narrative

An employee was using a handheld shovel to level out a ditch for a retention pond pipe. At the same time, an off-road truck was being loaded beside the retention pond. A rock (2 to 3 inches in diameter) then fell off the dump truck and rolled downhill into the ditch, striking the employee. The employee suffered back and left side injuries.
