Narrative

An employee was moving a pallet so it could be loaded by a forklift. The employee was utilizing a strap attached to the pallet when the strap broke, causing the employee to fall backwards into the forks of a fork truck. The employee suffered three fractured ribs on the right side, a bruised spleen and liver, and a laceration to the left ear, requiring hospitalization.
