Two employees were trying to loosen a bolt to disassemble a coupler between an engine and compressor. One employee was on the engine side while the other (the injured employee) was on the compressor side holding a shim. When the employee on the engine side turned the bolt for the coupling assembly, the engine crank rolled and amputated the injured employee's left middle fingertip.
