Narrative

An employee was using a tool to unjam a grinder machine when the grinder grabbed the tool and pulled his right hand between the tool and the framework of the grinder, de-gloving his hand and smashing his right ring fingertip. His fingertip was amputated. The machine was not locked/tagged out at the time of the incident.
