Narrative

An employee was operating a setting machine when a load of green brick became jammed in the machine. The employee turned the safety keys to set the machine to safe mode, removed the key, and attempted to unjam the machine. The machine then cycled with the employee inside, causing two tibia fractures and a puncture wound to the ankle.
