Narrative

An employee was running casing down. He was leaning with one hand on a slip and one hand on a guide bar that guides the casing down when he reached up and grabbed a roller without realizing it. The roller then rolled over and crushed his right pointer, middle, and ring fingers. Two fingers were amputated.
