Narrative

An employee was installing a new side head grinder on a planer machine. He was attempting to straighten the alignment of the machine's lifting shaft when the grinder freed from its position, dropped, and trapped the employee's left middle finger between the lifting shaft and a bolt head. His left middle finger was crushed, and the end joint was amputated.
