Narrative

A temporary employee was entering a trench that was 4 feet by 8 feet and 4 feet deep to help install a propane gas tank. While entering the trench, which had wood and aluminum siding installed, the employee slipped and fell. The siding then lacerated his left forearm, requiring hospitalization and surgery.
