On July 10, 2018, an employee was unloading a tandem dump truck at a dump site. The locking mechanism on the truck's gate was stuck. As the employee tried to unjam it, their hands were caught between the gate and the truck bed. The employee's left-hand fingers were crushed, with a possible amputation to the left middle finger; the right-hand fingers suffered contusions.
