Narrative

An employee was in a trench, cleaning off a duct about 8 feet away. A steel plate slipped while being placed in the trench; it knocked another steel plate (which had already been placed into the trench) against the employee and pinned him against the duct. He suffered a broken pelvis and femur and was hospitalized.
