Narrative

An employee was placing a weight on top of a mold. The weight slipped off, striking the employee on the left foot. The employee then hit his head against the metal flask surrounding the mold. He suffered a crushing injury to the foot and a fracture to the forehead and sinus area, and was hospitalized.
