An employee was laying standing seam metal roof decking when he stepped on insulation that was laid out underneath the metal decking and fell 30 feet through the insulation to the ground, landing on a crushed stone surface. He suffered a fractured left arm, fractured pelvis, and diaphragmatic hernia. A safety lanyard was not attached at the time of the incident.
