Narrative

On August 6, 2018, at approximately 12:16 PM, a hydraulic line located at the 3,000-ton press shear station unexpectedly released pressurized hydraulic fluid. The hydraulic fluid traversed to a separate operation next to the press and landed on a hot part (small ring expander), resulting in a flash fire. As a result, two employee suffered second and third degree burns to their hands and face, requiring hospitalization.
