Narrative

An employee was setting up a part on a machine that laminates PVC windows. He was picking out a piece of lamination film (vinyl laminate) left over after being trimmed off a piece of PVC window extrusion profile when his right middle finger entered the operating drive roller. As he pulled his finger out of the roller, it pulled off his right middle fingertip at the first joint.
