Narrative

Two employees were pushing a one-person vertical manlift onto a chair elevator. The lift had heavy-duty castors on the bottom. When the manlift entered the elevator, it shifted away from one of the employees, causing him to lose his balance and fall forward into the lift. In striking the lift, he dislocated and cut his right elbow.
