Narrative

On August 15, 2018, at approximately 10:30 a.m., an employee was attempting to clear a jam in the feeding section on a box former machine when the employee's gloved hand was caught by the machine's shuttle carrier, causing injuries that required surgical amputation of the middle finger on the employee's left hand.
