Narrative

An employee was unloading the back of a triaxle truck when the tailgate would not lock due to excess material in the bed of the truck. He placed his left hand on the side of the tailgate and his right hand on the latch to open the tailgate. The tailgate opened briefly and then closed immediately, catching his ring and middle fingertips. As a result, his left middle fingertip was amputated.
