Narrative

An employee was troubleshooting a pneumatic valve located in the control cabinet of a jet mill. The employee had removed the presence sensor cable and was proceeding to remove the presence sensor when the pneumatic valve actuated and caught his right middle finger, resulting in a fingertip amputation.
