Narrative

On August 17, 2018, at approximately 12:15 PM, an employee was unloading a dairy truck. The employee had one foot on the truck trailer gate and one foot on the loading dock plate. The truck moved forward without notice and the employee fell through the area, breaking a leg and requiring hospitalization.
