An employee was on a 5-foot tall scaffold painting above pipes while holding onto another pipe when the pipe broke and the employee lost his footing, falling to the concrete floor. He hit his head and landed on his shoulder, suffering a scalp hematoma, fractured shoulder, and fractured vertebra. The scaffold was unguarded at the time of the incident.
